Background

Duly Health and Care, formerly known as DuPage Medical Group, is one of the largest independent, multi-specialty physician-directed medical groups in the nation. With over 1,000 primary care and specialty providers across more than 140 clinics in the Midwest—primarily in Illinois—the organization delivers comprehensive, patient-centered care. Rebranded in September 2021, Duly emphasizes a promise to practice healthcare as it was originally intended: holistic, accessible, and focused on helping patients flourish. Their services span primary care, specialties like cardiology, immediate care through 10+ centers, and preventive wellness, all supported by a shared Epic electronic health record (EHR) system to ensure seamless coordination across sites.

In the realm of cardiac care, Duly manages a significant patient volume, including remote and in-clinic monitoring for thousands of individuals with implantable cardiac devices. Prior to partnering with Rhythm Management Group, Duly relied on the PaceArt system for these services across four key locations. Three locations handled approximately 3,300 patients through outsourced monitoring, while the fourth managed 3,000 patients internally for both in-clinic visits and remote transmissions. However, as their PaceArt contract neared expiration—with less than 90 days remaining—Duly faced mounting challenges: outdated workflows, fragmented in-clinic and remote processes, and the need for a more intuitive, cost-effective platform that integrated fully with their shared Epic instance.

Duly's cardiac monitoring operations were inefficient and costly under PaceArt. The system required separate handling for in-clinic visits at service-provided locations, leading to repetitive tasks, delays in data processing, and concerns over user licenses and escalating software costs. With all locations sharing a single Epic EHR, any new solution needed to support bidirectional integration for patient demographics, schedule management, report uploading, and charge capture—without disrupting daily operations.

The impending contract end added urgency: Duly required a rapid data migration of over 20 years of historical records, while ensuring minimal downtime for patient care. They sought a partner capable of delivering a unified platform that could handle both remote and in-clinic transmissions seamlessly, allowing clinical teams to prioritize patient outcomes over administrative burdens. Key evaluation criteria included an intuitive user interface for report generation and signing, robust analytics for real-time insights, and a no-cost model to eliminate additional software expenses.

After evaluating options, Duly selected Rhythm Management Group's Synergy platform for its proven track record in cardiac remote monitoring transitions, intuitive design, and flexible service model. Unlike competitors, Rhythm offered a no-cost platform for multi-location clients, bundling it with comprehensive monitoring services. This allowed Duly to outsource remote monitoring for three locations (3,300 patients) while using the platform only for the fourth location's internal team (3,000 patients)—all under one unified system.

Implementation

Rhythm moved swiftly to meet the tight timeline. The transition began with a comprehensive data migration from PaceArt, transferring over 20 years of patient records without loss or corruption. Simultaneously, Rhythm's engineering team built and tested a custom Epic integration, covering patient demographics, schedule management, report uploading, and charge capture.

The entire process—from planning to go-live—was completed in just eight weeks, well ahead of the PaceArt contract expiration. Implementation involved minimal disruption: Rhythm provided hands-on training for Duly's clinical and IT teams, including access to an ongoing support portal. For the service-provided locations, Rhythm assumed full remote monitoring responsibilities, while the internal team at the fourth location adopted the platform for in-clinic and remote processing. Post-go-live in July 2025, Rhythm offered unlimited user licenses and real-time support to ensure smooth adoption.

While currently not in the Epic Showroom with our integration as this one still used our sister company SpinSci’s integration to launch with the aggressive timelines, Duly will be our first client that moves to Rhythm’s standalone Epic FHIR instance by October 2025.
